1. Complete this form the same day that you received your package/s.
2. Do NOT throw away any packaging that the damaged item/s came in, including the packing material.
3. Ensure that you have pictures of all damages, pictures of the item in its original packaging (on the pallet if applicable), and a digital copy/picture of the packing slip/bill of lading.
